Moreover, brain metastases and superior vena cava syndrome (SVCS) in lung cancer patients further have been coupled with dismal prognosis and short overall survival. Such patients presenting in fourth stage of cancer have been considered usually incurable and considered for palliative treatment or sometimes Best Supportive Care. Case Report- We present a case of carcinoma lung with SVCS, who later on developed brain metastasis in the course of treatment. He has been successfully treated with radiation therapy to lung lesion with concurrent chemotherapy followed by adjuvant chemotherapy. He also received External Beam Radiation for brain metastasis. He is disease free after 7 years of treatment completion and on regular follow-up till date. It is restricted to the lungs and occurs in the elderly. We present a case of a 71 year old female, with progressive shortness of breath and dry cough. High resolution computed tomography (HRCT) of the chest revealed unilateral IPF of the right lung. She also had a grade 3 hiatus hernia, which only fuels the speculation of GERD being an important risk factor for the development of IPF. There are various methods exist as treatment option for reconstruction following tumor resection. Wide excision is the management of choice, but this creates a defect at the proximal end of tibia.We here present our experience with wide resection and vascularised autogenous fibula grafting for GCT of proximal tibia. From 40 years old male with GCT right proximal tibia with right knee flexion contracture. We preferred ipsilateral vascularized fibular graft as reconstruction modality following by arthrodesis on affected limb. Evaluation of anatomical, functional, radiological outcomes of this management was undergone. After 3,5 years , the patient has good outcome. There is no pain, no reccurency of tumor, the graft is union and become hyperthrophy. The estimated incidence of this innocuous condition is only about 0.2% in cytological material. It is of utmost importance to differentiate collagenous spherulosis from atypical proliferations and malignant mimics especially adenoid cystic carcinoma to avoid unnecessary treatment. Pathological diagnosis confirmed vulvar epithelium epithelioid sarcoma. No adjuvant treatment was given.
